婴幼儿先天性髋关节脱位功能性治疗不良结局原因分析

论证。目前,功能性治疗被认为是先天性髋关节脱位患儿的有效治疗方法,其主要原则是逐步实现股骨头在髋臼内的居中,并在保持髋关节和膝关节活动的同时,借助支具固定下肢。然而,在治疗过程中可能出现并发症,如复位后股骨头无菌性坏死及关节不稳定。这些并发症可导致严重且不可逆的改变,促进早期发育性髋关节骨关节炎的形成,并可能造成患儿残疾。对导致上述并发症发生的因素进行分析有助于提高治疗效果,但相关问题目前研究不足。
目的。揭示并分析婴幼儿发育性髋关节脱位功能性治疗不良结局的主要原因。
方法。本研究基于56例既往诊断为先天性髋关节脱位、年龄为1~3岁患儿的临床检查结果及其家长问卷调查资料。所有患儿均在居住地门诊条件下接受过功能性治疗。排除标准包括:合并神经系统疾病、致畸性髋关节脱位及病理性髋关节脱位。主要研究组为26例治疗结局不良的患儿,对照组为30例未发生并发症的患儿。问卷调查采用预先设计的调查表,内容涉及疾病的及时诊断情况及门诊阶段医疗服务质量。对所得数据进行了统计学处理。
结果。结果表明,门诊阶段功能性治疗失败的主要原因包括:家长未遵守支具使用方案、在治疗过程中长期忽视患儿的不适与焦躁表现、未能完整执行医嘱,以及未按时接受骨科医师的随访检查。
结论。结果表明,在采用功能性方法治疗先天性髋关节脱位患儿时,并发症的发生在很大程度上取决于社会因素。门诊骨科医生在治疗过程中重视并纠正上述因素,有助于减少并发症的发生,并在多数情况下避免外科手术干预。
