Pathogenesis of diabetes mellitus

Abstract

The article deals with a very important problem - the incidence of diabetes mellitus in the population. Epidemiological research on the incidence of diabetes mellitus reveals that the highest number of patients are in the age group of 40-49. Furthermore, the actual insidence of type II diabetes among the employable pop­ulation is 3-4 times higher than the registered one. The total number of diabetes mellitus patients in the Russian Federation has doubled in the last 15 years. We present the results of the recent decades of research on the etiology and pathogenesis of diabetes mellitus.

Current concepts of breast cancer prevention in patients with proliferative disorders of the reproductive system

Abstract

The review discusses the current concepts in estrogen-dependent proliferation, with relation to the molec­ular markers of proliferative disorders. We emphasize the rationale for wider use of the invasive diagnostics in the assessment of the mammary glands status. The data on breast cancer prevention in patients with benign proliferative disorders of the reproductive system is summarized. The use of ovarian suppression dur­ing reproductive period (GnRH agonists) and aromatase inactivation (aromatase inhibitors) is substantiated. The mechanisms of action of selective estrogen receptor modulators used in breast cancer treatment and prevention are discussed. The designs and results of all phase III-IV breast cancer chemoprevention trials are reported.

Molecular aspects of prostate cancer as basis for new methods and approaches for diagnostic and treatment of the disease

Abstract

There are not accurate and foolproof markers of prostate cancer. Such commonly used clinical methods as transrectal ultrasonography, determinations of PSA value and histopathologic analysis of biopsy are not enough sensitive and specific at an early stage. Furthermore, there are no available prognostic markers that will be able to predict the behavior of detected tumor and to make decisions about the most effective treat­ment of patients with prostate cancer. In this respect, the discovery of new markers with increased specifici­ty and sensitivity must be done. A molecular understanding of prostate cancer development is an important step toward this studies. During the past decade using the current methods of functional genomics the genes involved in oncogenesis and progression of prostate cancer had been identified and characterized. The can­didates for new molecular biomarkers and gene-targets for therapy of prostate cancer may be among these genes. In this articles we try to present the recent available dates concerning the molecular aspects of prostate cancer.

Effect of amino acids and antibodies to nerve growth factor low-affinity receptors on the lymphoid tissue culture

Abstract

The effect of L-amino acids in concentration 0.05 ng/ml was investigated in organotypic tissue culture of spleen in rats at age 3 months old. The high molecular and low hydrophobic amino acids - asparagine, lysine and glutamine acid are active, inducing more intensive growth zone in explants, as compared to control at the presence of the antibodies to nerve growth factor (NGF) low-affinity receptors (NGFRp75), being the apop­tosis inductors. In the contrast another group of low molecular and high hydrophobic amino acids which was active, inducing a less intensive growth zone and more p53 expression inhibition, produced at NGFRp75 presence the delay of the inhibition effect. These amino acids can mediate the apoptosis-induced effect by means of NGFRp75. The data obtained can be used by the synthesis of the novel bioregulator peptides.

Delayed development of functional interactions between cortical areas of the brain as the pathogenetic basis of residual neurological disorders in children

Abstract

The article describes the results of many years of research on the development of monosimptomatic residual neurological disorders in 3-11 year old children based on cross-correlation EEG analysis. We report the data on EEG analysis in speech disorders and related disorders (stammering, general speech underde­velopment, disgraphy), in attention deficit hyperactivity disorder (ADHD), enuresis and tic disorders, as well as in combinations of the above mentioned pathologies. Based on comparative EEG analysis in over 4000 patients and healthy children, we conclude that delayed development of functional interactions between brain structures, initially caused by minimal brain disfunctions in the perinatal period, underlies the pathogenesis of children's mental disorders. The specific features of such interaction impairments and their dynamics can be clearly revealed by cross-correlation EEG analysis. This work is important since no similar analysis of chil­dren's monosimptomatic residual neurological disorders is available in literature.

Abstract

The study group consists of 85 patients in age from 47 to 92 years. The significant for including was dis­tal or multilevel arterial occlusion. The patients who by two groups divided had been treated by basic med­ications with (basic group, n=45) or without (control group, n=40) Perftoran. Both patient groups were com­parable in age and grade of illness. Noninvasive hemodynamic evaluation by Doppler ultrasonography with calculation of ankle brachial index (ABI), systemic inflammatory evaluation by assay of WBC count, absolute neutrophil count, protein C (PC) level, lactate level, fibrinogen level, furthermore control of ache grade was performed on all patients before and after the treatment.

We demonstrated that use of Perftoran is a effective treatment method for patients with nonrecon- structible distal limb occlusion.

Abstract

Many angioneurologists study etiology, pathomorphology of the brain blood circulation insufficiency almost 100 years. The problem of stroke has left far from the neurology field and became an interdisciplinary problem and a subject of fundamental researches. New methods of diagnostics, medical treatment and reha­bilitation of stroke patients have appeared as a results of these researches. The Neurology Disease Chair of the Medical Military Academy has long traditions of the clinical, scientific and experimental works, and occu­pies one of the main places in the development and the decision of the stroke problem.

Human genome in a remote period after low doses irradiation

The current status of the problem of genetic consequences of low doses ionizing radiation on humans is presented. The special attention spared to the cytogenetical disturbances in Chernobyl clean-up workers.

The data received gives us the basis to say, that the low doses irradiation have been the starting mech­anism for the chains of events, led to genome instability in irradiated people.

Subintimal angioplasties in the treatment of critical ischemia of the lower extremities

Abstract

An increasing number of centers throughout the world are gaining experience with subintimal angioplas­ty. Promising results have been reported and the application of the technique has been expanded to include the iliac and crural arteries. The technique of subintimal angioplasty and several variations are discussed. Although primary patency rates compared with bypass are relatively low for patients undergoing subintimal angioplasty, limb salvage rates remain high. When a subintimal angioplasty fails, it frequently does so with­out the recurrence of symptoms, especially when a gangrenous lesion or ulcer has healed. Given the many advantages of this technique, which include reduced anesthesia requirements, a minimally invasive approach, and potential reductions in length of stay and cost, subintimal angioplasty will continue to have a role in the treatment of lower extremity ischemia in the treatment of critical limb ischemia.

Cytogenetic findings in patients with chronic lymphocytic leukemia

Abstract

Cytogenetic indexes in bone marrow and peripheral blood cells of 26 patients with CLL were explored. No changes in bone marrow cells were detected. In peripheral blood clonal changes were find in 17 pathients (65%), including trisomy 12 (18,5% from total number patients), rearrangements in chromosome 14 (7,7%), different abnormalities in 4q (18,5%). Clone with additional X-chromosome was detected in 18,5% of patients which may be considered as main or as additional clone in CLL.

The number of clonal cells differes from 2% to 75% depending on the form and stage of the desease. PHA and IL were equally efficient in cells stimulation. Obviously the frequency of chromosomal abnormalities depends on the quantity of analysed cells.

Morphogenetic function of lymphocytes and its clinical application

Abstract

The variation of different lymphocytes subsets in blood of Chernobyl clean up workers and cancer patients were compared with variation of rates of morbidity or mortality during their life. The direct and reverse relationships of rates of morbidity or mortality with the number of circulating CD4+Leu8+ T cells were esti­mated for Chernobyl clean up workers and cancer patients (with advanced renal and ovarian carcinomas both). The found reciprocal medical consequences of the same immature cells' subset can not be explained in general immunologic way and may be solved just in terms of morhpogenetic function of lymphocytes sup­ported a renewing normal and malignant tissues as well.

Carotid endarterioectomy in elderly patients

Abstract

The early results of surgical treatment of 127 patients with atherosclerotic lesions of carotid arteries are analyzed. The comparative study of the results is represented. All patients were divided into two groups. The first group consisted of 64 pts older than 60 years. The second consisted of 63 pts younger than 60 years. All patients underwent to 137 operations - carotid endarterieectomy. Severe stenoses and contralateral lesions were observed in second group more frequently (p<0.05). Ischemic heart disease (p<0.001), lower extremities atherosclerosis and COPD (p<0.05) were prevalent among the concominant pathology. Above mentioned peculiarities determined the necessity of a surgical strategy optimization in older group of patients. The classic carotid endarterioectomy with autoarter al plasty (50% of operations performed) in conjunction with the conduction analgesia are preferable. Using this technique allowed us to achieve good results, low frequency of postoperative strokes in older group of patients (1.5%) and absence of lethal outcomes.

Significance of detecting antithyroid antibodies in patients with newly diagnosed toxic goiter

Abstract

The paper represents the results of dynamic investigation of the levels of antithyroid antibodies in female cases of untreated Graves’ disease: the high level of autoantibodies to thyroid-stimulating hormone receptor found by prior examination and the lack of any decrease in the level of this autoantibodies during treatment indicates a high risk of acute condition following thyrostatic therapy.
